  1. Identify the document:

    1. Is the document lawful?
    2. Do you have the authority to witness it?
  2. Identify any special requirements such as personal knowledge of the person for a minimum period.
  3. Check that it is in the correct format.
  4. Identify if is to be sworn, affirmed or declared.
  5. Ensure the signatory is named in the document.
  6. Obtain proof of identity.
  7. Ensure dates are consistent throughout.
  8. Check for alterations, unanswered questions, omissions, spaces and material you know to be false. [Note that Victim Assistance Queensland financial application, domestic and family violence application and some family law documents permit unanswered questions.]
  9. Check annexures are referred to within the body of the document, are consecutively numbered or lettered and accompanied by a Certificate of Exhibit where necessary. See affidavit menu item for endorsement of annexures.
  10. Issue perjury warning.
  11. Administer oath, affirmation or declaration.
  12. Document signed in front of JP(Qual). If already signed, rule through and initial the unwitnessed signature. Signer must also initial the amendment.
  13. Sign and date the document, apend your full name, affix the seal and insert registration number.
  14. Complete the log.
